2.  Take a printout of the form.

3.  Sign the form as parents/guardians.

4.  Submit the form along with the following documents,

a. Birth Certificate/B.Form (original & Copy).

b. Copy of ITS cards (child and both parents).

c. Two passport size latest photographs with blue background of the child and both the parents in Libas ul Anwar.

d. Copy  of CNIC (both parents)

e. Vaccination Record/Card
